Community based healthcare and facilities aid in facilitating care and support for the aging population through a combination of at home evaluations and treatment and with clinic diagnostics and examinations. Home health is a pure example of community-based healthcare where a nurse, physical therapist, nursing aid, or provider can visit a patient at home to provide the same care a patient will be receiving in the hospital. This method avoids crowding in ED’s and in and inpatient acute care settings (unless necessary) and gives individuals comfort, management of chronic conditions, and the prevention of hospitalizations (Ellis, 2017). Furthermore, it decreases the stressors of having to find a way to visit the physician in a clinic.
Since the COVID-19 pandemic, the World Health Organization (2020) focuses on community-based healthcare by establishing clinics near areas that need assistance with care, especially for those with limitations in supplies and movement, fear, and misinformation. These clinics are serviced by trained healthcare professionals that are either paid or volunteer. It is also campaigned for and supported by non-governmental organizations, faith leaders, and community groups. The outcomes of community healthcare clinics aim to prevent communicable diseases, administer vaccines, assess for hypertension and hyperglycemia, control vectors and establish treatment, avoid exacerbations of chronic conditions, and maintain treatment regimens. Altogether, the goal is to take measures to protect the vulnerable population.
A doctoral prepared nurse can make an educated assessment on the patient’s needs while at home and can establish home health visits and care coordination if needed. Furthermore, the DNP nurse can aid in facilitating community healthcare clinics in areas where care is suboptimal. The DNP nurse is a leader that can convey the importance of care outside of a hospital setting and ca lead a group of healthcare workers to volunteer and support clinics to care for not only the aging populations, but also young children and pregnant women that have a challenging time obtaining healthcare.
